This registration form is for one person only who is registering for one or more courses.

If you would like to register and pay by credit card at the same time, you may use the SCSI register and pay form instead of this form.  This form is for registration only, not actual payments.  If you are registering (only) using this form,  you should contact the SCSI Registrar by phone (310-517-8844), FAX (310-540-0532), or e-mail to coordinate payment.

Please provide the following ordering information:  [When entering courses please use the course designation letters instead of the entire course name such as "AAI" instead of "Aircraft Accident Investigation."  To register for "SMS" show course designation and course dates you have selected.  For example,  SMS  2-13 Feburary 2004.  To register for a symposium, simply state the name of the symposium.  For example,  Brussels Symposium]
