If you would like to register and pay by credit card at the same time, you may use the SCSI register and pay form instead of this form. This form is for registration only, not actual payments. Please provide the following ordering information: When entering courses please use the course designation letters instead of the entire course name — for example "AAI" instead of "Aircraft Accident Investigation." Please include the course dates you have selected. For example, SMS-W 3-7 May 2010.
